REQUEST A VOTE-BY-MAIL BALLOT / UPDATE YOUR BALLOT REQUEST

Use our Online Vote-by-Mail Ballot Request Form below to make a ballot request for yourself or for an immediate family member. Whether you plan to be in town or out of town during the next election, voting by mail is an easy and convenient way to cast a ballot. Our office will send your ballot wherever you need it to be. The deadline to request that a Vote-by-Mail ballot be mailed to you is no later than 5 p.m. on the 12th day before Election Day.

To request a Vote-by-Mail ballot, you must provide:

  • Full name of voter

  • Date of birth

  • Residence address 

  • Mailing address, if different from home address (Ballots cannot be forwarded.)

  • Florida Driver License number, Florida ID number, or the last four digits of your Social Security Number 

  • Signature of voter (written requests only)

You are allowed to request a Vote By Mail ballot for yourself or for an immediate family member. Immediate family members include: spouse, grandparent, parent, child, grandchild, sibling, spouse's grandparent, spouse's parent, spouse's child, spouse's grandchild and spouse's sibling.

Your Vote-by-Mail ballot will include instructions on the proper way to mark and return your ballot. Read those instructions carefully. If you need a replacement ballot, contact our office at (239) 252-VOTE (8683).
