The information on this form will be used on our office's website, social media and during voter outreach and education events. After completely filling out the below biography form, you can upload a photograph of the veteran. If you are completing this form out for yourself, thank you for your service!

Photos can also be submitted to the Supervisor of Elections office via email at: ElectionWorker@CollierVotes.gov; or by mail/in person at 3750 Enterprise Avenue, Naples FL  34104. If mailing, please provide a copy of the photo rather than the original.
